Invaluable Charicter Creation Tool.

This is a sheet I got at a screenwriting class and I must say it is the single most useful story tool I have ever found.
Fill out all the questions for any reoccurring character. While most of the information will never come up it forces the writer to give a character depth and helps to get inside their heads.
I have added an additional segment at the end for fallout characters that is more mechanical than creative but still handy.

Not quite as essential as the above, but here are a few more helpful worksheets:
Character concept. Fill it out and then do something else for about ten minutes. Then come back and read it, or better yet have someone else read it. If you or your reader says "I would like to know more about this character." or the like you are good, otherwise try again.

3-7 Sentence Story. Summarize your story in the limited number of sentences then reread. if the summary would be sufficient to make you want to read the full story move on, otherwise go back and try again.

Structural Synopsis. Useful for the entire story and on a chapter by chapter basis. Used to help with plot and tone.

Okay, so I'm trying to use that character sheet you've got there, but quite a few of the points there make me go "Lolwut?".
For example, "Dress" and "posture". Not to mention "Most like what animal?". A pony, d'uh? xD

But yeah, you said you modified this from a sheet you got from a screenwriting class, so I can safely assume you know what you are doing here. Or atleast, more than me. xD
Could you explain what you mean by those points?

Another point that needs explanations; "How does he see friends"? I don't get what the sheet wants with this one.

Nope, not pony. That would be like answering human on the original form. You know how some people are compared to a cat or gorilla? Same concept.

Not everyone sees their friends the same way. Some view them as subservient (Celestia/Luna), some as a team, others as tools (to a degree I feel that Twilight falls in this one), as support (Dash), as protection (Fluttershy) as a source of favors (Rarity) and so on. Most folk still have quite a few letters to send Celestia if you catch my drift.

Another couple of questions; What do you mean by "class" in the first part of the sheet? As in for example "upper class" or "academic"?
And, what is an "unconscious action"? And while on that subject, what do you mean with "clean up"? Actually cleaning something? *being clueless*

Oh, yeah, and what's the deal with the question "Where does he/she usually wake up? "
Why is that question there? What is it's function? Why does anyone need to know that? xD
The only thing I can come up with as a proper answer for that is "Usually where he fell asleep". xD

Unconscious action probably means an "instinctive" action. Like if someone threw a rock at you what would your reaction be? To try and catch the rock or to dodge it?

Yeeah, I figured as much myself. Unfortunately, one of the listed reactions is "seduce", and I doubt you can seduce a flying rock.
Not to mention "Clean up" was one of them too. You'd have to be mighty fast to clean a flying rock.

A'yep. That would be social class. Wastlander would be an applicable answer.

How do they instinctively handle stressful situations. do they fight (directly oppose) hide (try to avoid the threat), deal (make sacrifices to remain safe), Clean up (ignore it and try to make there life more orderly to cope), seduce (try to make their tormenting force like them) this is whatever the characters base instincts tell them to do, not what they actually do.

How frequently dose BJ wake up where she gos to sleep? It actually says a good deal about a character whether they sleep in a bead, on the couch, or in the street (for the wasteland I would be more along the lines of, sleeps in the dirt, puts together crude bedding or insists on finding a prewar ruin for a bed).
